When you've tried therapy before and it didn't quite fit

Most of the people who come to my consulting room have tried therapy before. They’ve done a few sessions of generic CBT through the NHS; they’ve seen a counsellor who was kind but didn’t quite get the specifics – the ADHD that makes a routine feel impossible, the rejection sensitivity that turns small slights into days of rumination, the BPD pattern where intimacy and panic arrive together, the OCD ritual that doesn’t look like an OCD ritual until you notice you’ve been doing it for years.

What follows are the conditions I see most often, with the modalities and clinical training that match each one. None of this is a programme. Every one is a starting point for a course of work shaped around you.

ADHD

Neuroaffirming therapy for adult ADHD: executive function, rejection sensitivity, emotional dysregulation. CBT, DBT skills, and clinical experience with AuDHD profiles.

Addiction

Therapy for substance and behavioural addictions — alcohol, drugs, gambling, compulsive behaviours. DBT distress-tolerance skills alongside underlying-cause work.

Anxiety

Evidence-based treatment for generalised anxiety, panic, social anxiety, and health anxiety. CBT and DBT-informed approaches that work with, not against, the anxious mind.

Autism & Neurodiversity

Neuroaffirming therapy for autistic adults and AuDHD profiles. Strengths-based, no masking required, culturally aware. Specialist experience with late-diagnosed adults.

BPD

DBT-led treatment for emotional dysregulation, abandonment fears, identity instability, and the relational patterns that follow. The single most evidence-supported approach for BPD.

Depression

CBT, behavioural activation, and psychodynamic therapy for persistent low mood, loss of pleasure, and chronic self-criticism. NICE-recommended treatment, tailored to you.

Low Self-Esteem

Therapy for chronic self-criticism, perfectionism, imposter feelings, and the harsh inner critic that runs commentary on every decision. Compassion-focused and CBT-based.

OCD

CBT and DBT-informed therapy for OCD, including the less-visible forms. for example Pure-O, relationship OCD, and contamination checking, in line with NICE guidelines.

Parenting Issues

Therapy for parenting overwhelm, co-parenting conflict, the rupture-and-repair cycle, and the way your own childhood shapes how you parent now. Adolescent specialism.

PTSD & Trauma

Trauma-focused CBT and psychodynamic work for PTSD, complex PTSD, and unprocessed trauma. Paced carefully – no exposure before stabilisation.

Meet Sabbir: UKCP-Registered Psychotherapist

My name is Sabbir Ahmed, and I’m a UKCP-registered psychotherapist with over 20 years of clinical experience across the NHS and private practice. I trained at King’s College London and the Tavistock & Portman NHS Trust, and I work from Harley Street and City Road as well as online, UK-wide.

I don’t fit you to one programme. Rather than picking a modality first, I look at what’s actually happening for you and build the work around that, drawing on CBT, DBT, psychodynamic and compassion-focused approaches in whatever combination fits best to you.

How we work together

A simple 3-step process to lasting change

Step 1

Understand

Complimentary discovery call

A 20-minute no-commitment initial conversation. We talk about what’s bringing you to therapy, what you’ve tried before, and whether we’re the right fit. If we are, we book your first session.

Step 2

Build

Initial assessment & formulation

In the first 1–2 sessions we build a working understanding of what’s happening – your history, current patterns, and the goals that would make therapy feel worthwhile. I share my early clinical formulation and we agree the shape of the work.

Step 3

Grow

Strengthen & sustain

Weekly 50-minute sessions, with progress reviews every 6 to 8 sessions. Most clients see meaningful change within 12–20 sessions, although deeper work can take longer. You set the pace; I bring the structure.

Common questions

Frequently asked questions

You don’t need a formal diagnosis to start. Most people who come to me have a sense that something isn’t working, for example anxiety that feels disproportionate, a relational pattern that keeps repeating, or a low mood that won’t lift, without necessarily knowing what to call it. Our first conversation is partly about naming what’s happening. If we discover that what you need is outside my specialism, I’ll say so and point you to a colleague who fits better.

I work privately rather than through insurance panels. Some clients are reimbursed by their private health insurer (Bupa, Aviva, AXA, WPA, etc.) for sessions with a UKCP-registered psychotherapist. I can however provide invoices and any documentation needed for if your employer pays part, or all, of the cost of therapy, for example.

It depends on the condition and the depth you want to work to. Anxiety, low mood, and skill-building work often deliver results within 12–20 sessions. Trauma work, BPD, and longer-standing patterns typically run 6–18 months. I review progress with every client at 6–8 session intervals, ensuring that the work has a clear shape, not an open-ended one.

Sometimes, yes. If you’re working on a clinical issue (anxiety, depression, BPD) and also want focused work on something behavioural (productivity, confidence, life transitions), some clients do both with me, in separate session formats. We discuss whether this is the right approach during the discovery call.

Therapy for ADHD addresses the underlying emotional regulation, rejection sensitivity dysphoria, and trauma-adjacent patterns that often come with the diagnosis. Coaching focuses on practical executive function, routines, and goal achievement. Many adults with ADHD benefit from one, the other, or both at different points. My practice is not an emergency service. If you’re in crisis, please call 999, NHS 111, or the Samaritans on 116 123. Once you’re safe and stable, I’m available for medium- and longer-term therapeutic work.
