Contract Form

This contract outlines the terms and conditions for our therapeutic work together. Please read the following information carefully. By starting therapy, you agree to these terms, which ensure a safe, professional, and confidential environment.

1. Confidentiality

I adhere to the Code of Ethics of the UKCP (United Kingdom Council for Psychotherapy) to ensure your privacy and confidentiality. What you share during our sessions will remain strictly confidential.

Exceptions to Confidentiality:
I may need to break confidentiality if:

  • I believe you or someone else is at serious risk of harm.
  • There is a legal requirement to do so (e.g., child protection concerns or a court order).

I will always aim to discuss this with you first whenever possible.

2. Privacy Policy

  • Your contact details will be stored securely on my computer or mobile phone for the duration of our work together.
  • You may request to have your information deleted at any time.

3. Supervision

As part of professional practice, I attend regular supervision sessions to ensure the quality and effectiveness of therapy. These discussions are also confidential, and no identifying information about you will be shared.

4. Record-Keeping

I maintain brief written notes of our sessions for reference and to track therapeutic progress. These records are stored securely and are accessible only to me.

5. Prohibition of Session Recording

Recording therapy sessions—whether in-person or online—is strictly prohibited under the UKCP Code of Ethics.

  • Unauthorised recording breaches confidentiality and professional ethics.
  • Any violation may result in termination of therapy and potential legal action.

6. Complexity of Therapy Practice

If therapy evolves into areas beyond the agreed scope of practice, I reserve the right to end our sessions. Should this occur, I will discuss my concerns with you and recommend alternative services or specialists to better support your needs.

7. Timekeeping and Attendance

Consistency is essential for effective therapy. Sessions are scheduled at the same time each week to ensure continuity and progress.

  • Frequent cancellations or missed appointments without prior notice may lead to discontinuation of therapy.
  • If scheduling conflicts arise, please communicate as early as possible.

8. Cancellations and Payment

  • Please provide at least 48 hours’ notice if you need to cancel or reschedule a session.
  • Cancellations made with less than 48 hours’ notice will be charged in full.

Acknowledgement and Agreement

By continuing with therapy, you acknowledge and agree to these terms.
